Non-infectious postoperative complications remain a major cause of morbidity after coronary artery bypass grafting (CABG), contributing to prolonged hospitalization, delayed recovery, increased healthcare costs, and unfavorable cardiovascular outcomes. Current risk stratification systems, including EuroSCORE II and the Society of Thoracic Surgeons (STS) Risk Calculator, are primarily based on clinical and operative variables and therefore do not adequately reflect the marked interindividual variability of the perioperative immune-inflammatory response. Increasing evidence indicates that dysregulated inflammatory and endothelial activation plays a central role in the pathogenesis of postoperative complications; however, reliable prognostic models integrating these biological mechanisms remain limited. This study was therefore designed to develop and validate an immunoinflammatory prognostic model capable of improving early prediction of non-infectious postoperative complications following CABG.
